Weekend Registered Nurse (PRN)

The RN provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Responsibilities include creating, implementing, executing, and coordinating the overall plan of care to promote wellness and prevent rehospitalization; maintaining continuity of patient care by exchanging information with physicians, agency personnel, patients, families, and community resources;

conducting initial and/or comprehensive nursing evaluation visits; developing the plan of care within state guidelines; ensuring patients meet home health eligibility and medical necessity guidelines; maintaining accurate documentation and clinical notes; establishing an efficient visit route; and providing high‑quality clinical services within the scope of practice from admit through discharge.

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Ability to reliably commute to multiple patient residences daily within the assigned geographic area.
