Licensed Practical Nurse - Skilled Nursing & Rehab

Bethany Center for Rehabilitation and Healing in Goodlettsville, TN is seeking a compassionate LPN to provide bedside nursing care to residents in our skilled nursing and rehab facility. The LPN will administer medications and treatments, monitor and document resident conditions, support care plans, and collaborate daily with RNs, therapists, and physicians. You will educate residents and families, uphold infection control and safety standards, and help residents reach their highest level of function.

Responsibilities

• Provide bedside nursing care to residents in a skilled nursing and rehabilitation setting
• Administer medications and treatments in accordance with physician orders and facility policy
• Monitor, record, and report changes in resident condition to RNs and physicians
• Assist with development and implementation of individualized care plans
• Maintain accurate, timely documentation in the electronic health record (EHR)
• Support infection control, safety, and fall prevention protocols
• Collaborate with an interdisciplinary team including RNs, therapists, and physicians
• Educate residents and families on care plans, medications, and wellness strategies
• Assist with admissions, discharges, and care transitions
• Participate in ongoing training and quality improvement initiatives
