What is a PACE Program?

The P Program of All-inclusive Care for the Elderly (PACE) is a unique health care model designed to help seniors (participants) stay safely in their home for as long as possible. PACE coordinates and provides or facilitates all medical care for its participants. Services are delivered across various settings, including on site at the PACE center, in the community, and in participant homes.

Clinic Registered Nurse (RN) is responsible for delivering participant care utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation through evidenced-based practice in accordance with established policies, procedures, and scope of practice.

Clinic RN duties and responsibilities include, but are not limited to:

  • Assesses participants and collects data; completes required forms with appropriate responses according to Program standards; identifies participant’s overt problems/needs and sets priorities; identifies problems requiring further referral and/or follow-up; interprets and records latest diagnostic results; performs advanced nursing assessment using critical thinking skills.
  • Develops a plan of care based on the nursing process which incorporates the plans of other disciplines and continuing care needs; includes the participant / family in developing or revising the plan of care; makes referrals to interdisciplinary support services. Initial and periodic assessments (minimally every 6 months) to be completed prior to the scheduled team meeting. Communicates participant changes to team members.
  • Care provided conforms to accepted practice standards; provides correct treatments/procedures and other care as prescribed and according to participant care standards; demonstrates understanding of age-related characteristics and needs of patients served; documents nursing procedures in appropriate forms (i.e., electronic medical record); provides participant and family education in coordination of 24 hour care delivery; identifies emergency situations and independently initiates advanced therapy per protocol;

    acts as coordinator of participant care with other health care personnel and evaluates participant care measures instituted; understands and demonstrates respect for participant’s rights and utilizes established mechanisms for management of ethical issues in patient care.
  • Records participant care delivered as planned and any variation, with appropriate rationale; makes and records observations related to impending or associated problems; implements nursing measures related to impending or associated problems.
  • Using the nursing process, delegates nursing care to appropriate personnel; integrates cost effective measures into nursing practice; recognizes participant problems and takes responsibility initiating care measures and for documenting the same; demonstrates active participation in quality improvement (QI) processes; complies with Program expectations to meet staffing demands based on participant care needs; complies with policies addressing safe working conditions; monitors unsafe working conditions;

    recognizes inappropriate and/or ineffective participant care management, resolves issues/problems and completes reports as required; fulfills on-call requirements as assigned and adjusts staffing for census (volume) and acuity.
  • Participates as a member of the Interdisciplinary Team (IDT). Attends staff meetings and takes part in participant care planning.
  • Provides oversight as needed to licensed and unlicensed personnel.
  • Assists with and performs necessary tasks associated with care provided to participants in the Clinic and occasionally in the home.
  • Carries out provider orders related to participant care.
  • Other duties as assigned.
