Nurse Practitioner Or Physician Assistant
We are seeking a Nurse Practitioner or Physician Assistant to provide high-quality, patient-centered care to residents in skilled nursing facilities throughout Mt. Pleasant, MI. This role offers clinical autonomy, flexible scheduling, and the opportunity to make a meaningful impact in post-acute and wound care.
Key Responsibilities- Conduct on-site rounds and comprehensive patient assessments.
- Diagnose and manage acute and chronic conditions, including wound care needs.
- Prescribe medications and develop individualized treatment plans.
- Address acute and emergent clinical issues.
- Communicate effectively with facility nursing staff via phone or secure messaging.
- Maintain timely, accurate clinical documentation using a company-provided iPad.
- Maintain reliable on-site attendance and comply with clinical, facility, and regulatory standards.
- Part-time opportunity with flexible scheduling.
- Facility shifts are typically 6 hours, with rounds generally beginning between 7:00–10:00 a.m.
- No on-call or overnight requirements.
- iPad with LTE provided for mobile charting.

- Clinical experience in post-acute care, skilled nursing, wound care, or a related setting preferred.
- Qualified backgrounds include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine & Rehabilitation, and Geriatric Medicine.
- Strong clinical judgment, communication skills, and commitment to patient-centered care.
