Join a compassionate team dedicated to providing comfort, support, and responsive end-of-life care when patients and families need it most. The After Hours On-Call Registered Nurse provides skilled nursing support, symptom management, and crisis intervention to hospice patients receiving care in home and facility settings during evenings, weekends, and after-hours coverage while collaborating closely with the interdisciplinary team to ensure compassionate, quality care.
Schedule, Hours & Territory Full-time dedicated on-call role with a rotating 7-days on/7-days off schedule
Coverage hours are 4:00 PM to 8:30 AM and the full weekend
Mt. Pleasant, MI and up to a one-hour radius surrounding

Key Responsibilities Triage patient and caregiver calls after hours
Provide clinical guidance and symptom management
Complete urgent in-person visits as needed
Coordinate care with interdisciplinary team members
Document all assessments, interventions, and communication accurately and timely in the EMR HCHB (Homecare Homebase)
Active Registered Nurse (RN) license
Hospice field experience preferred
Strong clinical judgment and decision-making skills
Experience using HCHB (Homecare Homebase) preferred
Field Role Requirements Active driver’s license
Ability to pass a criminal background check
Ability to pass a Motor Vehicle Record (MVR) check
Current TB testing in accordance with agency policy
Current CPR certification as required for the role
