The Care Team Hospice is seeking a driven, relationship-focused Community Care Liaison (CCL) to grow our presence in the community. • Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.
• Serve as a dedicated advocate for hospice by educating the community and sharing the value of comfort-focused, patient-centered care.
• Achieve and exceed census and admission targets by positioning our hospice as a preferred provider within the community.
• Engage regularly with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well-being.
• Lead hospice discussions with patients and families, clearly communicating services and guiding the consent process with professionalism and empathy.

Bachelor’s degree or equivalent

• 1-2 years healthcare specific outside sales experience.
• 1-2 years of Hospice or Home Health sales Preferred.

Ability to perform regular travel within assigned geography and extended geography for training and meetings.

Must have reliable transportation. Valid and current driver’s license and current auto insurance required.
