Essential Duties and Responsibilities- Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
- Establishes open lines of communication with consultants.
- Uses effective interviewing skills to collect information from patients, families, and staff members.
- Assists the Director with patient or family interviews for assessment purposes.
- Assists the Director with evaluation of each patient’s condition and medical data to determine special needs or the need to amend the admission assessment.
- Applies comprehensive knowledge of nursing resource material and incorporates relevant information into care plans.
- Establishes open lines of communication with interdisciplinary team members to develop a comprehensive and holistic patient care plan.
- Maintains schedule for assessment/care plan meetings and coordinates participation with nursing center staff, family, therapists, and others.
- Ensures patient assessment and care plans are completed within a timeframe that is in accordance with current regulations.
- Assists the Director with coordinating the development of a care plan (preliminary and comprehensive) that identifies problems/needs, appropriate interventions, and measurable goals.
- Facilitates education to inform associates of the patient’s care plan, reviewing the plan before care is administered.
- Coordinates the review of each assessment/care plan for accuracy, timeliness, appropriate signatures, and dates.
- Monitors the MDS process for accuracy according to the MDS guidelines and other accepted standards.
- Assists the Director in education of the interdisciplinary team and other staff members in the MDS process.
- Assists the Director with the physical assessment of patients to ensure accurate MDS process as requested.
- May assist with nursing service educational programs related to patient assessment and care.
- May assist with meeting all shifts for the purposes of planning, coordinating, and implementing patient care.
- Assists in compliance with Federal, State, and local regulations in relation to scheduling, performing, and implementing patient care plans.
- May be required to assist with management reports and participate in special assignments.
- Complies with specific time limitations in terms of assistance given to the Director for scheduling and planning patient care.
- Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
- Contributes to committees, work groups, project management, and other collaborative efforts within the System.
- Performs other duties as necessary to ensure the success of the System.
- Proficient with computer applications, especially Microsoft Office, Excel, and PowerPoint.
- Excellent organizational and prioritizing skills.
- Effective and professional verbal and written communication skills.
- Communicates warmly and friendly with patients, families, and visitors, displaying a helpful, caring demeanor.
- Responds to concerns in a professional manner and answers questions appropriately.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
- Ability to write routine reports and correspondence.
- Ability to speak effectively before groups of customers or associates of the organization.
- Ability to add, subtract, multiply, and divide in all units of measure using whole numbers, common fractions, and decimals.
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram format.
- Ability to deal with problems involving several concrete variables in standardized situations.
- Proficiency with Microsoft Outlook, Microsoft Excel, and Microsoft Word.
- Nursing degree from an accredited college or university, or graduate of an approved RN program.
- Active RN License to practice in the State of Georgia.
- Required to sit, talk or hear regularly; frequently walk; use hands to finger, handle, or feel and reach; occasionally stand.
- Vision requirements include close vision, distance vision, and the ability to adjust focus.
