Upperline Health is seeking a Nurse Practitioner (APP) to provide clinic‑based care primarily for the GA market at the Murphy Hardeeville Clinic. In collaboration with the local physician team, the APP develops complex plans of care, delivers medical diagnostics and treatments in clinic, patient homes, or via telehealth, and educates patients and families on medication usage, diet, nutrition, and crisis anticipation & prevention.
WhatYou’ll Do
- Develop and implement clinical plans of care for adult patients with chronic conditions (e.g., diabetes, COPD, CHF, debility, dementia).
- Gather patient history, perform physical examinations, and conduct diagnostics in clinic, via telehealth, or in the patient’s home.
- Create and execute treatment plans aligned with patient goals of care and current conditions.
- Collaborate with the interdisciplinary team to address patients and families’ physical and psychosocial needs.
- Provide compassionate care to complex patients and their families.
- Educate patients and families on medication usage, side effects, illness progression, diet & nutrition, and medical adherence.
- Coordinate with Upperline clinical team members and community providers to optimize care and resources.
- Communicate with local physicians to review patient records and care plans promptly, in compliance with state regulations and company policy.
- Maintain patient medical records and documentation consistent with state regulations and company standards.
- Complete accurate and timely documentation per risk adjustment standards.
- Participate in weekly interdisciplinary team meetings and scheduled educational sessions.
- Offer telephonic support for escalated calls from triage nurse.
- Engage in continuing education as required by the state Board of Nursing.
- Prescribe medication as permitted by the state Board of Nursing.
- Perform other related duties as deemed necessary.
- Master’s degree in nursing (required).
- Board certification as an Adult, Adult‑Geriatric, or Family Nurse Practitioner (required).
- Current state Registered Nurse & Nurse Practitioner license (required).
- Basic Life Support (BLS) certification (required).
- Minimum six months experience as a nurse practitioner in Primary Care/Internal Medicine.
- Value‑based care experience preferred.
- Experience in podiatry, orthopedics, wound care, radiologic interpretation, ABIs, or injections is a plus.
- Possession of DEA registration or eligibility.
- Comfortable providing care in patients’ homes.
- Previous experience managing, supervising, or coaching clinicians highly preferred.
- Reliable automobile, valid driver’s license, and minimum state-required liability auto insurance (required); mileage reimbursement provided.
- Adaptability to thrive in a fast‑paced, growth‑forward environment.
- Experience working in an EMR—Athena Health EMR highly preferred.
- Proficiency using Microsoft Office products.
- Excellent oral and written communication skills to ensure effective and meaningful interactions with patients, family caregivers, clinicians, and team members.
