Job Description

The LPN is responsible for delivering nursing service duties, in accordance with Healthcare Provider orders while providing oversight to the resident care and Medication Management Program in accordance with healthcare provider orders, state regulations and SRG policies.

Our community setting is a relaxing change from a hectic hospital. Here at SRG you have a chance to get to know your patients; our residents count on the LPN to provide compassionate care. You will provide residents with nursing services in accordance with physician’s orders. The LPN is responsible for the administration of medications and treatments.



Essential Duties & Responsibilities :
  • Work as part of the team within the Health Services Department
  • Participates in and completes resident’ assessments to identify resident care needs.
  • Develops and updates resident service plans which include person centered approaches and interventions to determine resident care level.
  • Follows up on residents’ health conditions, and /or changes of condition with healthcare providers, residents and responsible parties.
  • Communicates relevant resident information to healthcare providers, alerts community staff and responsible parties of care concerns and service plan changes.
  • Performs nursing services as directed and in accordance with the Nurses Practice Act, and state regulations.
  • Develops and maintains effective relationships and communication with residents, families, responsible parties and other healthcare providers.
  • Collaborate with healthcare providers by reviewing weekly visit notes and ensuring continuity of care.
  • Assist with coordination of care for new move ins, residents on extended leave of absence and/or residents returning from alternate levels or receiving services from a new health care provider.
  • Completes documentation via Yardi EHR according to SRG policy and state regulations.
  • Initiate internal incident report and state reports as needed. Participates in internal incident investigations as needed.
  • Coordinates residents’ incidental medical, dental, vision, hearing, podiatry care and/or telehealth visits as needed.
  • Coordinate onsite lab diagnostic testing services and/or durable medical equipment as needed.
  • Completes Long Term Care insurance verification forms as needed.
  • Supports the Medication Management Program with a focus on the med room systems for medication delivery services, medication storage, and security.
  • Collaborate with preferred pharmacy partners.
  • May provide residents with ADL care and assistance with medication administration as needed.
  • Assist the Director of Assisted Living with daily staffing issues as needed.
Qualifications
  • Must be a Licensed Practical Nurse and/or Registered Nurse holding a valid nursing license in good standing.
  • Minimum of one (1) year of experience in a senior living environment preferred
  • First Aid and CPR certification required
  • Must pass a criminal background clearance before beginning work
