Job Summary:
The Field Registered Nurse (RN) serves as the case manager for the Home Health Team, providing high‑quality nursing care to assigned patients and directing Home Health Aides in delivering exceptional patient care. The RN is responsible for assessing patient and family needs, developing individualized care plans, and ensuring the best possible recovery and rehabilitation outcomes. This role requires adherence to the scope of practice as defined by the licensing authority and compliance with agency policies and standards.
Key Responsibilities:Patient Care & Case Management:
- Conduct comprehensive initial and ongoing assessments of patients’ physical, emotional, and environmental status, including home safety and family support capabilities.
- Develop, implement, and evaluate individualized care plans in collaboration with physicians, patients, and families.
- Provide skilled nursing care, including administering medications, treatments, and rehabilitative services as prescribed by the physician.
- Monitor and document patients’ conditions, progress, and responses to treatments, reporting any significant changes to the physician and Nursing Supervisor promptly.
- Educate patients and families on disease management, care techniques, and preventive measures to promote optimal health outcomes.
- Act as the primary case manager for all nursing and therapy care cases.
- Supervise and support Home Health Aides, ensuring adherence to care plans and agency standards.
- Conduct supervisory visits to Home Health Aides at least once every two weeks.
- Collaborate with paramedical personnel to coordinate comprehensive patient care.
- Maintain accurate and timely clinical records, including admission notes, progress reports, and care plans.
- Ensure proper certification and recertification of the Plan of Care.
- Adhere to agency policies, HIPAA guidelines, and confidentiality protocols.
- Submit progress reports to physicians for patients with changing conditions or deviations from the Plan of Care.
- Participate in weekly meetings with the Nursing Supervisor to discuss patient care, policies, and planning.
- Assist in orienting new staff and contributing to in‑service training programs.
- Maintain a flexible weekly itinerary, prioritizing patient needs, visit durations, and geographic proximity.
- Notify the office of daily itinerary changes and maintain availability for contact during working hours.
- Ability to speak, read, and write in English or Arabic.
- Capable of lifting, transferring, or assisting patients weighing up to 160 pounds, with or without mechanical devices.
- Ability to stand, walk, climb stairs, bend, twist, and squat for extended periods (up to 80% of the workday).
- Comfortable working in environments with potential exposure to blood or infectious body fluids, utilizing provided protective equipment.
- Ability to concentrate on detailed tasks despite frequent interruptions.
- Strong organizational skills to follow and complete daily routines and responsibilities.
- Capable of comprehending and applying professional education materials.
- Emotional resilience to cope with the mental and emotional stress of the role.
- Graduate of an accredited School of Nursing with a valid RN license in the state of Maryland/Virginia.
- Minimum of 1 year of nursing experience, preferably in community health or home health care.
- Thorough knowledge of nursing principles, procedures, and patient care techniques.
- Excellent communication, interpersonal, and problem‑solving skills.
- Ability to meet the physical and health requirements of the role.
