Receives patient care assignment from the registered nurse in charge and initiates delegated tasks.;
Collects and documents data for admissions and reassessments and communicates changes to the RN/LPN.;
Contributes to safety and quality care by relaying information via charting and communicating to team members.;
Monitors patients, takes vital signs, assists with baths, ambulating, feeding, skin care, and personal and oral hygiene.;
Prepares patients for discharge and accompanies them upon dismissal as indicated.
