Nurse Practitioner - NP - Hospice and Palliative Care

Job Overview

At SSM Health, we are seeking an experienced Nurse Practitioner (NP) to care for patients in the community as part of the Advanced Illness Management program in Southern IL. Inquiries can be sent to

• Schedule:
  
  Monday-Friday, 8 a.m.–5 p.m. NO nights/weekends/holidays or call.
• Community-based program: build rapport with patients in an intimate setting.
• Mileage reimbursement available.
• Practicing autonomously with virtual support from a tight-knit team of collaborating physicians and peer APPs.
• Comprehensive financial package: experience‑based compensation, benefits within first 30 days (health insurance, retirement plan + match, PTO + more).

• Graduate of nationally accredited Nurse Practitioner program.
• Current NP licensure in Illinois.
• Ability to work autonomously.
• Experience in palliative care, hospice, or geriatrics required.

Compensation based on experience, specialty, labor market, and other qualifications. Posted pay range reflects what SSM Health reasonably expects to offer. Position may also be eligible for incentive pay. All benefits begin within 30 days.
