SSM Health in Illinois is seeking a Nurse Practitioner to join the Advanced Illness Management (AIM) program, delivering patient care in the community. The NP will work autonomously while receiving virtual support from a collaborating physician and peer APPs.
This role offers flexible PRN hours (20-30 hours/month), no on-call, mileage reimbursement, and exposure to hospice/palliative care in a compassionate setting. Strong clinical judgment and geriatrics experience are preferred.
