At SSM Health, we are committed to delivering exceptional healthcare services. Our Southern IL region is actively seeking an experienced Nurse Practitioner (NP) to care for patients in the community as part of our Advanced Illness Management (AIM) program.
Responsibilities and Schedule- PRN - Flexible Hours - 20 - 30 hours per month (you determine schedule)
- NO call!
- Community-based program – build rapport with patients in an intimate setting
- Mileage reimbursement available
- Performing visits in a 2-week time frame to comply with the provided guideline
- Understanding of hospice and palliative environment experience is preferred
- Enjoy practicing autonomously with virtual support from a tight‑knit team of a collaborating physician and peer APPs
- Graduate of a nationally accredited Nurse Practitioner (NP) Program
- Current NP licensure in Illinois
- Ability to work autonomously
- Experience in palliative care, hospice, or geriatrics required
Compensation for this role is based on a variety of factors including relevant experience, medical specialty, labor market, and other qualifications. The posted pay range for this position is what SSM Health reasonably expects, in good faith, to offer based on the circumstances at the time of posting. This position may also be eligible for incentive pay. SSM Health may ultimately pay more or less than the posted range as permitted by law.
