Job Summary:

The Westchester Center for Rehabilitation & Nursing, a leading provider of rehabilitative therapy and long-term care, is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team! As an LPN at our state-of-the-art facility, you will play a vital role in delivering high-quality patient care and supporting our mission to enhance the lives of seniors in our community.

Licensed Practical Nurse LPN Responsibilities:

• Provide direct patient care, including administering medications, wound care, and daily living activities

• Monitor and report patient status to the Registered Nurse (RN) and other healthcare professionals

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Participate in quality care initiatives and quality improvement projects

• Maintain accurate and detailed records of patient care and treatment

• Collaborate with other healthcare professionals to ensure seamless transitions of care

• Provide emotional support and education to patients and their families

Licensed Practical Nurse LPN Requirements:

• Current LPN license in the state of New York

• Prior experience in a long-term care or rehabilitation setting preferred

• Strong communication and interpersonal skills

• Ability to work efficiently and effectively in a fast-paced environment
