The Westchester Center for Rehabilitation & Nursing is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our weekend team in Mount Vernon, New York. As a cornerstone of the local healthcare community, we provide high-quality rehabilitative therapy and long-term care to seniors in need.
About the Licensed Practical Nurse LPN Role
As a Licensed Practical Nurse, you will play a vital part in delivering exceptional patient care to our weekend residents. Working closely with our interdisciplinary team, you will be responsible for:
Providing routine nursing care to patients, including vital sign monitoring, medication administration, and wound care
Assisting patients with daily living activities, such as bathing, dressing, and grooming
Maintaining accurate and up-to-date patient records
Collaborating with therapists and physicians to develop and implement personalized care plans
Participating in Quality Improvement initiatives to enhance patient outcomes and satisfaction
Licensed Practical Nurse LPN Responsibilities
Provide direct patient care, including medication administration, wound care, and vital sign monitoring
Assist patients with daily living activities, such as bathing, dressing, and grooming
Maintain accurate and up-to-date patient records, including medication administration records and progress notes
Collaborate with interdisciplinary team members, including therapists and physicians, to develop and implement personalized care plans
Participate in Quality Improvement initiatives to enhance patient outcomes and satisfaction
Contribute to a positive and inclusive work environment through excellent communication and teamwork
Licensed Practical Nurse LPN Requirements
Current LPN licensure in the state of New York
Prior experience in long-term care or rehabilitation settings
Strong clinical skills and knowledge of nursing practices and procedures
Ability to work effectively in a fast-paced environment and prioritize tasks
Excellent communication and interpersonal skills
