The Westchester Center for Rehabilitation & Nursing is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our weekend team in Mount Vernon, New York. As a cornerstone of the local healthcare community, we provide high-quality rehabilitative therapy and long-term care to seniors in need.

About the Licensed Practical Nurse LPN Role

As a Licensed Practical Nurse, you will play a vital part in delivering exceptional patient care to our weekend residents. Working closely with our interdisciplinary team, you will be responsible for:

• Providing routine nursing care to patients, including vital sign monitoring, medication administration, and wound care

• Assisting patients with daily living activities, such as bathing, dressing, and grooming

• Maintaining accurate and up-to-date patient records

• Collaborating with therapists and physicians to develop and implement personalized care plans

• Participating in Quality Improvement initiatives to enhance patient outcomes and satisfaction

Licensed Practical Nurse LPN Responsibilities

• Provide direct patient care, including medication administration, wound care, and vital sign monitoring

• Assist patients with daily living activities, such as bathing, dressing, and grooming

• Maintain accurate and up-to-date patient records, including medication administration records and progress notes

• Collaborate with interdisciplinary team members, including therapists and physicians, to develop and implement personalized care plans

• Participate in Quality Improvement initiatives to enhance patient outcomes and satisfaction

• Contribute to a positive and inclusive work environment through excellent communication and teamwork

Licensed Practical Nurse LPN Requirements

• Current LPN licensure in the state of New York

• Prior experience in long-term care or rehabilitation settings

• Strong clinical skills and knowledge of nursing practices and procedures

• Ability to work effectively in a fast-paced environment and prioritize tasks

• Excellent communication and interpersonal skills
