Job Overview

The Mental Health Administrator – Investigative Agent Supervisor oversees and manages the facility-wide investigative and incident management system. This role ensures timely, accurate, and compliant Major Unusual Incident (MUI) and Unusual Incident (UI) investigations, provides leadership to investigative staff, and maintains strong collaboration with internal and external partners. The Administrator serves as a key resource for maintaining regulatory compliance, identifying systemic risks, supporting corrective actions, and promoting a safe, accountable environment aligned with state and federal requirements.

The position requires strong analytical skills, sound judgment, and the ability to lead investigative operations with integrity, objectivity, and professionalism.

• Plan, direct, and evaluate the facility-wide investigative and incident management system, including MUI/UI investigations.
• Establish program policies, procedures, investigative standards, protocols, and performance expectations.
• Supervise investigative staff and manage unit operations, scheduling, and workload distribution to ensure 24/7 coverage.
• Analyze MUI/UI data to identify trends, systemic risks, and root causes.
• Communicate investigative findings to executive leadership and interdisciplinary teams to support risk mitigation and prevention strategies.
• Serve as primary liaison to the Ohio State Highway Patrol and other external entities on investigative matters.

• Monitor regulatory compliance through case reviews, report audits, and tracking systems within investigative processes.
• Ensure adherence to MUI rule requirements, Medicaid standards, and licensure expectations.
• Evaluate the quality, timeliness, and accuracy of investigations and written reports.
• Prepare investigative documentation and reports to support Medicaid and licensure surveys.
• Identify compliance risks and recommend or support process improvements to strengthen investigative practices and ensure audit readiness.

• Partner with executive leadership to develop Plans of Correction based on investigative findings, audit results, and survey citations.
• Coordinate implementation and follow-up activities related to corrective actions in investigative and incident management processes.
• Monitor the effectiveness of corrective actions to ensure sustained compliance and reduction of risk factors.
• Provide consultation to programmatic staff on incident-specific prevention strategies while maintaining objectivity and independence.

• Provide leadership, coaching, and performance management to investigative staff.
• Conduct training related to MUI rule requirements, investigative procedures, and internal reporting processes.
• Represent investigative services in executive meetings, committees, and interdisciplinary forums.
• Serve as Administrator on Duty as assigned.
• Participate in facility-wide committees and activities, ensuring alignment with investigative responsibilities.

For all “programmatic” positions, completion of undergraduate core program in one of following fields: mental health & developmental disabilities technology, social work or comparable field (e.g., child & family community services), special education, psychology or rehabilitation or 24 mos. exp. in development & implementation of treatment/ habilitation plans for persons who are mentally ill or developmentally disabled & working as part of interdisciplinary team to assess consumers'/residents' needs & abilities;

12 mos. trg. or 12 mos. exp. in state & federal regulations, standards & court orders governing care, custody, treatment, programming & environment of consumers/residents in mental health or developmental centers, or regulating drug treatment center operations or federally funded drug abuse services, or governing delivery of psychiatric services in adult correctional institution, or affecting community mental health programs (i.e., category of state & federal regulations, standards & court orders must be linked directly to type of facility & services…
