Responsibilities
- Assists with facilitating optimal health outcomes for patients and the community through the provision of evidence-based individual, group and/or community education.
- Provides diabetes education in an inpatient and an outpatient setting. Develops, implements and evaluates community and hospital patient education related to disease management; including but not limited to diabetes self-management education/training (DSME/T).
- Coordinates aspects of the DSME/T program at KCH, as accredited by the American Diabetes Association (ADA), including planning, implementation, and evaluation.
- Serves as a content expert and resource to physicians/providers, staff members, and the community.
- Responsible for establishing and maintaining the diabetes self-management education (DSME) accreditation/recognition program through the American Diabetes Association and/or the American Association of Diabetes Educators.
- Develops, implements and evaluates evidence-based education, including diabetes self-management education and facilitates optimal health outcomes for patients with or at risk for diabetes.
- Provides evidence-based education for all patients with diabetes.
- Provides diabetes education to staff members and serves as a consultant to all healthcare providers.
- Develops and directs diabetes education programs to serve the diabetes community.
- Assists to identify, develop, implement, and evaluate disease management programs including outreach programs and health screenings for the community.
- Adheres to organizational policies and procedures; regulatory accrediting body requirements; and professional practice standards.
- Graduate of an accredited program for nursing education, BSN preferred
- Current licensure to practice professional nursing in the State of Ohio
- Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required
- A minimum of 3 years’ experience in direct patient care preferred; 2 years recent experience in chronic disease care and education, behavior change strategies and program management preferred
- Teaching experience in a patient care, classroom and/or community setting preferred
- Certification as a Diabetes Care and Education Specialist preferred
