Location:

Mount Vernon, Ohio, a short 30‑minute scenic commute from Columbus, Zanesville, Lancaster, and Newark.

Responsibilities
  • Practice community‑based nursing and administer medical care.
  • Provide clients with basic medical information and promote healthy lifestyle changes including tobacco cessation, nutrition, physical activity, substance‑use prevention, and medication adherence.
  • Serve as liaison with physicians and other providers; coordinate with client’s physician, psychiatrists, and pharmacy regarding medical care and prescriptions.
  • Administer shots and daily medications following prescribed orders.
  • Complete nursing progress notes and annual nursing assessments.
  • Document client’s vital signs, including blood pressure, weight, BMI, waist circumference, and other data.
  • Work effectively as part of an interdisciplinary team to assess and refer clients to appropriate levels of care.
  • Maintain confidentiality of HIPAA and other sensitive information.
  • Maintain the highest level of integrity and professionalism in all job performance.
  • Stay abreast of current trends and best practices; attend required trainings on relevant topics.
  • Perform job responsibilities and maintain all records in accordance with BHP’s policies, procedures, regulatory authorities, and applicable laws.
  • Work assigned schedule, exhibit regular and punctual attendance, and work outside normal hours as required.
  • Perform additional duties as assigned.
Qualifications
  • Associate’s degree in Applied Science of Nursing.
  • Three to five years of nursing experience in a facility for individuals with mental health issues.
  • Licensed Practical Nurse (LPN) license and CPR/BLS certification required.
  • Strong written and oral communication skills and ability to collaborate with other service providers.
  • Basic computer, phone, and typing skills.
  • Ability to comply with ADA reasonable accommodations as approved by BHP.
