TITLE:
Mental Health Professional – Targeted Re-Entry Services Jail Services
Non-Exempt Position
RESPONSIBLE TO:Program Director
MAJOR DUTIES:Under the supervision of the Program Director and Program Supervisor, the Mental Health Professional delivers community-based behavioral health and care coordination services to participants. The position requires independent decision-making, effective care coordination, regulatory compliance, and collaboration with multidisciplinary teams to support participant recovery and successful community integration.
Core Competencies and Responsibilities
Clinical Assessment and Care Planning
- Conduct comprehensive biopsychosocial, behavioral health, and Health-Related Social Needs (HRSN) assessments consistent with Medicaid Re-Entry Demonstration requirements, agency standards, and applicable Washington State regulations.
- Utilize clinical judgment to identify participant strengths, barriers, risks, and service needs to support successful community re-entry.
- Develop, implement, and monitor individualized Re-Entry Care Plans that are person-centered, trauma-informed, culturally responsive, recovery-oriented, and strength-based.
- Regularly evaluate participant progress and modify care plans as clinically indicated.
Care Coordination and Service Delivery
- Provide targeted case management and behavioral health support services under Washington State's Medicaid Re-Entry Demonstration (1115 Waiver) and applicable managed care requirements.
- Coordinate transitions of care between correctional facilities, behavioral health providers, medical providers, housing resources, employment services, and community-based organizations.
- Facilitate participant access to behavioral health treatment, substance use treatment, medical care, benefits, transportation, housing, and other social support services.
- Demonstrate effective crisis identification, intervention, de-escalation, and referral practices consistent with agency protocols and participant safety needs.
Interdisciplinary Collaboration
- Function as an active member of a multidisciplinary treatment team by contributing clinical observations, recommendations, and service updates.
- Collaborate effectively with correctional staff, healthcare providers, probation officers, courts, managed care organizations, community partners, and other stakeholders to support continuity of care.
- Participate in case consultations, staffing meetings, utilization reviews, and treatment planning discussions.
