Position: Clinical Nurse Specialist/Nurse Practitioner - FT - Days - Heart and Vascular Institute OPS @ MV

Job Description

The Clinical Nurse Specialist/Nurse Practitioner is a registered nurse who, as a result of graduate education and in-depth clinical experience, possesses advanced knowledge and clinical skills necessary to provide expert nursing care. The major functions include education, consultation, clinical practice, research, and leadership. The primary responsibility is the direct application of clinical expertise to patients/families and patient care staff.

Key Responsibilities
  • Weekly patient rounding, coordination of care and communicating plan of care to the patient/family and multidisciplinary team.
  • Navigate the continuum of care for patients including the coordination of post‑op/procedure visits, Acute Rehab, Home Care and/or Skilled Nursing Facility (SNF).
  • Optimize post‑hospital care coordination including Tele‑monitoring and specialist referrals.
  • Collaborate with enterprise‑wide teams to improve patient satisfaction and patient care outcomes.
  • Liaison to Clinical Data Specialists of the applicable data registries.
  • Perform preliminary/concurrent case reviews to support cardiac quality assurance, quality improvement activities and outcomes management.
Community Outreach (Stroke Program)
  • Accountable for the accreditation and certification of the Stroke Program, including all supporting data, process improvement, and multidisciplinary evidence‑based practice development.
  • Coordinate with Medical Staff Stroke Program Director for maintenance and growth of the program; for evaluation of outcomes, and for improvement of outcomes.
  • Organize community screening events & lectures.
  • Consult with healthcare team and patient during hospitalization.
  • Post‑hospitalization follow‑up.
  • Develop or collaborate on patient teaching materials.
  • Secondary responsibilities include staff development regarding care of the Stroke patient and development of related materials and input on related PI projects.
Behavioral Health CNS Responsibilities
  • Coordinate activities for the Behavioral Health Services and the Medical Center.
  • Facilitate admissions between Behavioral Health Services and the Medical Center.
  • Consult to the Medical Center regarding psychiatric issues and substance abuse.
  • Ongoing staff education and support.
  • Lead and direct diagnosis‑based teams in the care and service of Behavioral Health patients across the continuum.
  • Promote effective and appropriate utilization of clinical, ancillary and material resources.
  • Define and achieve optimal clinical and resource outcomes, ensuring patients receive care that follows evidence‑based standards and organizational goals.
Qualifications
  • Master’s degree in Nursing.
  • At least 24 months clinical nursing experience in the area of specialty.
  • Demonstrated ability to produce results and coordinate projects.
  • Experience in quality improvement and evidence‑based practice.
  • Prior CNS, NP, or nursing education experience preferred.
  • Excellent verbal and written communication skills, problem solving, conflict resolution skills.
  • Computer skills that include word processing and database management.
License/Certification/Registration Requirements
  • Current California RN license.
  • California certification as a Clinical Nurse Specialist or California licensure as an NP.
  • BLS certification.
  • For Stroke Coordinator/Neurovascular – Three (3) years of recent experience in acute nursing and demonstrated clinical competence and in‑depth knowledge in the clinical area.
  • For Neonatal – Three (3) years of recent experience in neonatal nursing care, including at least one in a NICU equivalent to a Regional or Community NICU, and demonstrated clinical competence and in‑depth knowledge.
  • Certificate in neonatal Intensive Care Nursing from a nationally recognized accrediting organization.
  • Current NRP certification.
  • For Behavioral Health – Knowledge of utilization review, discharge planning, clinical pathways, managed care and “Best Practice Guidelines” preferred.
  • NCC or appropriate national credentialing certification in psychiatric/mental health.
  • National certification through ANCC or a national credentialing association preferred.
  • Certification in Nonviolent Crisis intervention Training or equivalent certification.
