Home First in Mountain View, CA, is seeking a Case Manager for the Cold Weather Shelter Program. This temporary, full-time role supports women and families during winter, providing case management, enrollment, safety planning, referrals, and crisis de-escalation to promote housing stability.
You will report to the Program Manager, work Mon-Fri, and document through HMIS, with the team collaborating with community partners to meet participants' needs.
