Job Title: Social Worker – MSW LSW Shift: Full Time | Monday-Friday 8:00 am - 4:30 pm Location: Ball Memorial, Muncie, IN
Position OverviewThe Social Worker MSW/LSW provides face‑to‑face assessments, counseling, and resource coordination for patients and families. This role involves collaborating with the healthcare team to ensure safe, timely, and comprehensive care, while addressing complex psychosocial, legal, and safety needs. The social worker serves as a key liaison in abuse/neglect cases, guardianship, foster care, adoption, and mental health placements, and supports patients through crisis situations with de‑escalation and intervention skills.
Key Responsibilities- Meet face‑to‑face with patients and families, conducting psychosocial assessments and counseling.
- Collaborate with all members of the healthcare team to facilitate patient care, education, and resource provision.
- Interpret and communicate complex patient and family needs; role‑model caring practices for other caregivers.
- Assess psychosocial risk factors, including prior functioning, support systems, reactions to illness, and coping skills.
- Counsel patients and families on emotional, social, and financial impacts of illness/disability.
- Access and mobilize community and family resources to meet identified needs.
- Conduct mental health assessments upon physician referral.
- Facilitate complex discharge planning and therapeutic interventions.
- Act as the primary liaison in cases of child abuse/neglect, elder abuse/neglect, institutional abuse, and domestic violence.
- Maintain communication and coordination with child and adult protective services (APS/DCS).
- Utilize principles of crisis intervention and de‑escalation techniques, supporting safety during acute crises.
- Provide intervention support for guardianship, foster care, adoption, and mental health Level II placements.
- Deliver specialized mental health services, including crisis intervention, addiction assessments, and psychiatric referrals.
- Support patients during dangerous or potentially violent situations, utilizing de‑escalation techniques.
- Conduct interviews with victims, perpetrators, families, and significant others.
- Provide intervention for end‑of‑life, guardianship, foster care, and adoption cases.
- Develop and maintain strong working relationships with hospital leadership, law enforcement, community agencies, schools, probation, and other partners.
- Respond appropriately in situations involving violence or threats, including on‑call and holiday rotations.
- Education: Master’s Degree in Social Work from an accredited institution.
- Licensure: Permanent Indiana LSW Licensure required.
- Experience: 2-3 years relevant healthcare, mental health, or community‑based experience preferred.
- Certifications: Certification in Crisis Prevention Intervention or similar de‑escalation training required within 6 months of hire.
- Knowledge &
Skills:- Evidence‑based psychosocial assessment, short‑term therapeutic counseling, and crisis intervention.
- Knowledge of disease processes and human behavior related to social and health needs.
- Complex discharge planning skills.
- Knowledge of state and local legal mandates for child and adult protective services.
- Principles of growth and development across the lifespan, including end‑of‑life care.
- Ability to identify decision‑makers and facilitate goals of care conversations.
- Experience working with diverse populations across geographic, cultural, and financial backgrounds.
- Strong interpersonal, communication, and collaboration skills.
- Proficiency in electronic medical records and documentation.
May be scheduled for on‑call and holiday rotations.
