Position Summary:
The RN Navigator serves as a primary point of contact for patients undergoing infusion therapy, providing care coordination, patient education, and support throughout their treatment journey. You will collaborate with physicians, nurses, social workers, and other healthcare professionals to develop personalized care plans, advocate for patients, and ensure timely delivery of treatments. Additionally, you will oversee and support staff in the infusion area, promoting efficient operations and delivering high-quality, patient-centered care.
Key Responsibilities:
- Coordinate and facilitate patient care plans across the outpatient infusion setting
- Educate patients and their families about treatment processes, expected outcomes, and follow-up care
- Collaborate with multidisciplinary teams to ensure timely and appropriate care delivery
- Oversee and support staff in the infusion room, ensuring adherence to policies and procedures
- Promote a positive, solution-focused environment, fostering optimism and teamwork
- Manage multiple priorities effectively, balancing clinical and operational needs
- Advocate for patients’ needs and preferences while considering care quality and efficiency
Qualifications:
- Requires an Associates of Nursing (ASN).
- Bachelors of Nursing (BSN) preferred.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.
- Requires Basic Life Support certification through the AHA.
- Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
- Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.
- Requires knowledge of personal computers using various types of word processing and database software.
- Requires clinical work experience, medical terminology.
- Requires understanding of medical record requirements, regulations and policies.
- Experience in infusion therapy is a plus, but not required.
- Prior leadership or supervisory experience preferred.
