The LPN will be responsible for providing nursing care to patients in the Brookside Care Behavioral Health LTC facility. The LPN will work under the supervision of a registered nurse and will be responsible for administering medications, monitoring vital signs, and providing basic patient care. The LPN will also maintain accurate patient records and communicate with other healthcare professionals as needed.
Key Responsibilities- Administer medications and treatments as prescribed by the physician
- Monitor and record vital signs
- Assist with activities of daily living, such as bathing, dressing, and grooming
- Assist with mobility and transfers
- Document patient care and progress in medical records
- Communicate with other healthcare professionals as needed
- Provide emotional support to patients and their families
- Adhere to all nursing standards and procedures
- Current LPN license in the state of Indiana
- Experience working in a nursing home or long‑term care facility
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong attention to detail
- Ability to prioritize and manage multiple tasks
- Basic computer skills
- Competitive hourly rate based on experience and knowledge. Starts at $34.00 per hour
