- Licensed Practical Nurse with current license in the state of employment.
- Minimum of one year of experience as an LPN in an acute care setting (two years preferred). Home‑care experience desired.
- Effective communication skills and the ability to create positive impressions with patients, families, physicians, and co‑workers.
- Calm, patient, accommodating, compassionate, and caring while working with patients.
- Knowledgeable in nursing best practices and able to make appropriate nursing judgments.
- Detail‑oriented, observant, disciplined, and able to prioritize and be timely.
- Able to follow directions and work as a team member.
- Valid driver’s license and auto insurance with your name listed as a driver.
- Provide skilled nursing care in a patient’s home as ordered by the attending physician.
- Follow order sets and goals made by RN and notify RNCM of any changes in patient condition.
- Administer medication as prescribed.
- Counsel patient and family on disease/injury processes and management.
- Incorporate patient and family in plan‑of‑care development.
- Teach patients preventative measures to reduce re‑hospitalizations.
- Listen to patients and family members.
- Complete all clinical documentation per agency protocol and Medicare/Federal guidelines.
- Maintain active LPN license in the state of employment.
- Communicate any changes with scheduler outside the normal schedule.
- Participate in on‑call requirements, case conferences, in‑service trainings, and mandatory agency meetings.
- Maintain a professional appearance and adhere to agency policies and procedures.
- Perform other duties as assigned.
- Drive to patient’s primary location per scheduled visit.
- Daily attendance at assigned visit locations.
- Document all aspects of visits within 24 hours to meet Medicare guidelines.
- Submit required documentation within specified company requirements.
- Provide skilled care within the scope of practice for an LPN.
- Complete LPN visits in assigned geographic location per patient need within federal and state specifications.
