Job Title: Licensed Practical Nurse (LPN) – Skilled Nursing / Long-Term Care Cardinal Care Strategies Muncie, IN
Position Type: Full-time / Part-time / PRN Position Summary:
The Licensed Practical Nurse (LPN) provides compassionate, professional nursing care to residents in a skilled nursing and long-term care environment. The LPN assists in planning, implementing, and evaluating resident care under the supervision of a Registered Nurse (RN) and in accordance with the Indiana Nurse Practice Act, facility policies, and state and federal regulations.
Key Responsibilities:Provide direct nursing care to residents, including medication administration, treatments, and documentation.
Monitor residents’ health status, observe for changes in condition, and report promptly to the RN or physician.
Assist with developing, updating, and implementing individualized care plans.
Supervise and direct Certified Nursing Assistants (CNAs) to ensure quality resident care.
Maintain accurate, timely, and complete medical records and documentation in accordance with facility and regulatory requirements.
Participate in admission, discharge, and transfer processes for residents.
Communicate effectively with residents, families, physicians, and interdisciplinary team members.
Promote residents’ rights, dignity, and independence.
Follow infection control, safety, and emergency procedures.
Participate in continuing education and staff development activities.
Current LPN license in good standing with the Indiana State Board of Nursing.
CPR certification (BLS) required.
Prior experience in skilled nursing or long-term care preferred.
Strong clinical, communication, and leadership skills.
Ability to work collaboratively as part of an interdisciplinary healthcare team.
Must be able to lift, move, and assist residents as needed.
Rotating shifts including weekends and holidays, as assigned.
Overtime and on-call duties as required by facility needs.
Frequent standing, walking, bending, lifting, and transferring residents.
Exposure to infectious materials and chemicals in a healthcare environment.
Ability to respond to emergency situations quickly and calmly.
