Gentiva is seeking a Hospice RN / RN Case Manager to provide compassionate, patient-centered care in patients’ homes or care settings in Indiana. You will perform comprehensive assessments, implement and document plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to deliver high-quality hospice services.
You will educate families, manage symptoms, and ensure regulatory compliance while supporting the patient’s and family's emotional and spiritual needs.
