- Verify the Plan of Care before each visit and provide care according to RN delegation, physician orders, assessment data, and established standards and guidelines
- Incorporate patient care goals from the Plan of Care into nursing visits and document them in skilled nursing notes
- Communicate significant findings, problems, and changes in patient condition or environment to the RN Case Manager, physician, clinical supervisor, and other involved personnel
- Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician
- Implement patient safety plans using patient, family, and community resources
- Teach patients appropriate self-care techniques while delivering quality care
- Suggest revisions to the Plan of Care and notify the RN Case Manager of identified patient care issues
- Provide substantial and specialized nursing services under the direction of a Registered Nurse or physician
- Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions
- Document medication regimens and update medication profiles
- Treat patients and caregivers in the highest and most effective manner
- Implement actions to prevent avoidable hospitalizations and ER visits
- Provide positive, supportive communication to physicians, patients, families, visitors, and agency personnel
- Report to the Clinical Team Manager
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations

Demonstrates expertise in patient care management, including the ability to implement care plans, communicate effectively with healthcare teams, and provide specialized nursing services under supervision. Proficient in documenting patient care and medication regimens while ensuring patient safety and quality care delivery.
