Hospice Registered Nurse (RN) Case Manager – Full-Time | Mundelein, Libertyville, Third Lake, IL

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered end-of-life care.

• Only 1 weekend every 12 weeks
• Focus on quality of care over visit volume
• Supportive leadership and interdisciplinary team
• Autonomy in the field without feeling isolated

• Monday–Friday, 8:30 AM – 5:00 PM
• Territory:
  Mundelein, Libertyville, Third Lake, IL
• $80,000 – $97,000 annually (based on experience)

• Manage a dedicated hospice caseload as the primary RN Case Manager
• Perform patient assessments, admissions, and individualized care planning
• Deliver skilled nursing care focused on comfort and dignity
• Coordinate care with physicians and interdisciplinary team members
• Conduct routine visits and adjust care plans as needed
• Provide end-of-life support, including family education and pronouncement
• Supervise Home Health Aides in compliance with regulations

• Graduate of an accredited RN program
• Current CPR and First Aid certification
• Hospice or home health experience preferred
• Strong clinical judgment and communication skills
• Valid driver’s license and proof of insurance
