Position Summary

Nights, 11pm – 7:30am – Assisted Living. The Staff LPN is responsible for implementing, directing, communicating, and documenting the nursing process and for providing a safe environment for patients, visitors, and associates. The LPN practices in accordance with current standards of care, the Nurse Practice Act, and the Powers Health policies and procedures. Assisted Living is ideal for individuals who require help with daily tasks.

The Hartsfield Village Assisted Living apartments (licensed by the State of Indiana) have nursing presence 24 hours per day, 7 days per week, to assist with activities of daily living. This is provided in a residential setting that is beautiful to the eye as well as the spirit.

Job Responsibilities
  • Documents a physical assessment each shift including subjective/objective data.
  • Communicates with patients, families, and/or significant others to individualize a patient’s plan of care.
  • Verifies that physician’s orders are transcribed properly and carried out promptly.
  • Reports significant changes in a patient’s condition to the unit manager, charge nurse, shift supervisor, and/or physician.
  • Continuously evaluates patient’s responses to plan of care, teaching plans, and rehabilitation activities throughout their stay and upon discharge.
Education / Experience
  • Graduate from an accredited school of practical nursing.
  • Currently licensed as an LPN by the State of Indiana Health Professions Bureau.
  • Obtains and maintains current certification in BLS, and other required certificates in specialty areas.
