Position: Alliance Liaison RN- Community - FT Day

Overview

The Alliance Liaison enhances continuity of patient care by collaborating with hospitals, physicians, hospice/home care, Home Medical Equipment Services, and patients and caregivers. The Liaison works with case management, social workers, physicians and others on the health care team to transition patients to the post-acute care setting, building and maintaining partnerships that enhance post‑acute care referrals to Alliance.

Responsibilities
  • Participate as part of the Health Care Team to support a collaborative effort to facilitate quality patient care.
  • Integrate and coordinate various aspects of service delivery, care facilitation, payer authorizations, service access, discharge and post‑discharge activities, and utilization review.
  • Ensure documentation is complete by reviewing patient orders, verifying insurance information, gathering clinical data, and determining patient care needs.
  • Identify patients that meet criteria for Alliance Services.
  • Facilitate the transfer of patients from hospital to home care/hospice services; consult with nursing, social services and other support services.
  • Promote and build strong relationships with referring physicians and hospital staff in the Case Management Department.
  • Provide information and education for health care team, physicians, hospital staff and patient/family; answer questions and requests and complete applicable reporting as required.
  • Effectively communicate in writing and orally with patients/families and other health team members.
  • Deliver and set up portable medical and related equipment to necessary facilities as assigned.
  • Ensure equipment is operating safely and that proper training is completed.
Education & Formal Training Requirements
  • Required:

    Associate's Degree in Nursing
  • Preferred:
    Bachelor's Degree in Nursing
Experience Requirements
  • Required:

    Registered Nurse with 3‑5 years of experience
  • Preferred:
    Experience in home care or hospice settings
Licenses & Certifications
  • Required:

    Registered Nurse, Tennessee;
    Tennessee Board of Nursing license
  • Required:

    BASIC LIFE SUPPORT certification from the American Heart Association
  • Preferred:
    Certification in a clinical specialty

  • Serve as a patient advocate by connecting with patients and families, listening to understand their needs, and honoring commitments.
  • Skill and proficiency in communications and interpersonal relations.
  • Skill and proficiency in applying highly technical principles, concepts, and techniques central to nursing practice.
  • Analytical skills necessary to assess complex patient conditions and develop comprehensive plans of care.
  • Capacity to synthesize multiple sources of information to assist in resolving complex problems.
  • Demonstrated ability to develop and maintain working relationships with physicians and collaborate with health professionals at all levels.
  • Demonstrated excellent facilitation skills.
  • Ability to plan, schedule tasks, and maintain control of own workflow.
  • General computer skills required; advanced computer skills preferred.
  • Use initiative, independent judgment, and critical thinking skills in decision‑making.
  • Work independently to set priorities, coordinate tasks, and complete work.
  • Knowledge of supplies and related equipment.
Supervision Provided by this Position

No supervisory or lead responsibilities are assigned to this position.

Physical Demands

The physical activities of this position may include climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion. The employee must have good balance and coordination. The position requires exerting up to 50 lbs. of force occasionally and/or up to 25 lbs. of force frequently. Close visual acuity, including color, depth perception, and field of vision, is required to perform activities such as assessing patients, preparing and analyzing data, transcribing, viewing a computer terminal, and extensive reading.

Frequent invasive and non‑invasive patient contact, exposure to patient body fluids, hazardous or poisonous materials, and the ability to react quickly to emergency situations are also required. Handling and administering narcotics, when necessary, is part of the role.
