Overview

Are there things harming your community that you feel passionate about changing? If the answer to any of these questions is "yes," this might be the service opportunity for you. PNs are trusted, knowledgeable frontline health personnel who typically come from the communities they serve.
AUCH is hiring a PN to work on the CHR team. The PN will work primarily with individuals living in medically underserved areas, assisting them with enrolling in cancer prevention programs, using Ask-Advise-Connect (AAC) to screen and address gaps in health literacy and Other Health Related Needs (OHRNs), and utilizing Motivational Interviewing (MI) to help patients identify where they are in the cycle of change, affirm internal motivations for change, and address any ambivalence related to healthy lifestyle changes.

- Contact a high volume of patients regularly and conduct follow-up outreach and additional MI sessions in English and Spanish.
- Establish trusting relationships with patients.
- Enroll patients in cancer prevention programs.
- Utilize MI to help patients identify where they are in the cycle of change, affirm internal motivations for change, and address any ambivalence related to healthy lifestyle changes.
- Document activities and outcomes in appropriate databases.
- Attend team meetings to discuss progress, successes, and challenges.
- Other duties as assigned by partners or supervisors must be in accordance with Ameri Corps terms and conditions.
