Direct Care Professional

Job description:

We are seeking a compassionate, reliable, and patient DSPD Caregiver to provide direct support to individuals with intellectual, developmental, or physical disabilities. The caregiver will assist clients in achieving greater independence, personal growth, and community involvement in accordance with their individual support plans (ISPs) and Division of Services for People with Disabilities (DSPD) guidelines.

Key Responsibilities:

• Provide direct care and daily living assistance, including personal hygiene, meal preparation, medication reminders, and mobility support.
• Support clients in achieving goals outlined in their Individual Support Plans (ISP).
• Encourage community involvement and assist clients in participating in social, recreational, and vocational activities.
• Teach and reinforce life skills such as communication, budgeting, cooking, and self-care.
• Maintain accurate and timely documentation of services provided, behavioral observations, and progress notes.
• Follow all DSPD regulations, agency policies, and health and safety procedures.
• Provide transportation to appointments, work, or community activities as needed.
• Respond appropriately to emergency situations and report incidents according to policy.
• Communicate effectively with team members, supervisors, clients, and families to ensure consistent, high-quality care.
