Utah Healthcare Corps - Center HOPE Patient Navigator

Patient Navigators (PNs) are frontline health personnel who help community members access healthcare. The selected PN, fluent in English and Spanish, will work on the Huntsman Center for HOPE team, primarily serving individuals in medically underserved areas.

• Contact a high volume of patients regularly and conduct follow-up outreach and additional Motivational Interviewing sessions in English and Spanish.
• Establish trusting relationships with patients.
• Enroll patients in cancer prevention programs.
• Use Motivational Interviewing to help patients identify their stage of change, affirm internal motivations, and address ambivalence regarding healthy lifestyle changes.
• Document activities and outcomes in the appropriate databases.
• Attend team meetings to discuss progress, successes, and challenges.
• Perform other duties as assigned by partners or supervisors in accordance with Ameri Corps terms and conditions.

• Permits attendance at school during off‑hours.
• Permits working at another job during off‑hours.

• Fluency in English and Spanish.
• High school diploma or GED.
• Availability for a nine‑month, 900‑hour term (25 hrs/week).
• Commitment to the Ameri Corps service‑learning model.
