Position Purpose

Assists in planning, organizing, implementing, and evaluating the overall operation of the facility’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, and the facility’s established policies and procedures.

Required Qualifications
  • Minimum of a high school diploma or its equivalent.
Major Duties and Responsibilities
  • Assist the Social Service Director/Consultant/Administrator in planning, developing, organizing, implementing, evaluating, and directing social services programs.
  • Meet with administration, medical and nursing staff, and other related departments to plan social services as directed.
  • Participate in discharge planning, development and implementation of care plans and resident assessments.
  • Encourage residents/responsible parties to participate in the development of their care plan and invite them to care plan meetings.
  • Assist residents in obtaining transportation to medical appointments upon discharge.
  • Accurately and completely document social service actions and interactions in each resident’s medical record.
  • Assist with facility policy development and annual review to positively impact the quality of care.
  • Ensure staff members are knowledgeable about residents’ Rights and encourage staff to maintain and enhance each resident’s dignity.
  • Engage in Advance Care Planning for assigned residents upon admission and regularly review Advance Directives with residents or representatives.
  • Advocate for residents and assist them in asserting their rights within the facility.
  • When an allegation of suspected abuse, neglect or exploitation arises, report to the Social Services Director and/or Administrator and assist with reporting to the appropriate State agency.
  • Ensure that residents who display mental illness or psychosocial difficulties have access to appropriate treatment and resources.
Additional Assigned Tasks
  • Treat all residents with dignity and respect. Promote and protect all residents’ rights.
  • Establish a culture of compliance by adhering to all facility policies and procedures and state/federal regulations and guidelines.
  • Follow appropriate safety and hygiene measures at all times.
  • Maintain confidentiality of protected health information.
  • Report noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.
  • Report any retaliation or discrimination to HR or compliance officer.
  • Participate in all life safety and emergency drills and trainings.
  • Report work‑related injuries and illnesses immediately to supervisor.
  • Follow established infection control policies and procedures.
  • Complete all assigned training and skill competency as a condition of employment.
  • Interview residents and/or their responsible parties to obtain the resident’s social history.
  • Make arrangements for residents to obtain items such as clothing and personal care supplies.
  • Assist residents in voicing and resolving grievances, reviewing complaints, and reporting actions taken to the Social Services Director.
  • Participate in Resident and/or Family Council as needed or requested.
  • Assist with, or arrange for, a resident’s communication of needs through the resident’s primary method of communication or in a language that the resident understands.
  • Coordinate Social Services activities with other departments as needed.
  • Perform administrative requirements such as completing assigned reports and submitting them to the Social Services Director.
  • Attend Care Plan meetings and, if requested, serve as a member of the QAPI Committee or the Behavior Management Committee.
  • Attend Department Head/Standup Meetings and mandatory in‑services.
