Registered Nurse Case Manager

We are looking for a qualified Hospice and Home Health RN Case Manager that shares our values and wants to assist in our vision to provide the excellent care that the community expects from us.

We pride ourselves on very low employee turnover. We are looking for a qualified Part-Time RN Case Manager who wants a long-term position with a great company. This position is flexible at 20-25 hours per week and is paid at a per-visit rate.

• Skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care.
• Create patient-centered goals with patients, family members and providers.
• Assess, supervise and direct patient care.
• Develop plans of treatment.
• Ensure chart compliance with state regulations.
• Coordinate patient care with physicians and other providers.

• Licensed as a Registered Nurse by the State of Utah.
• Current Utah driver's license and automobile insurance.
• CPR certification.
• Minimum 1 year nursing experience (hospice and home health experience preferred but not required).
• Strong documentation skills and ability to work in a home setting.
