Job Description

We are seeking a compassionate and dedicated Registered Nurse (RN) to join our hospice care team. In this role, you will provide holistic, patient-centered care to individuals with terminal illnesses, ensuring comfort, dignity, and quality of life during their end-of-life journey.

Responsibilities
  • Assess, plan, implement, and evaluate individualized care plans using the nursing process, incorporating advanced technology, therapeutic interventions, and evidence-based practices.
  • Coordinate care with an interdisciplinary team of healthcare professionals.
  • Support patients and families with guidance and emotional support during the critical time of caring for a loved one.
  • Lead the coordination and delivery of patient care in the home setting, overseeing the entire patient care journey from admission through discharge.
  • Plan education and collaborate across disciplines, fostering strong relationships with patients, families, and payors.
  • Ensure seamless communication, high-quality outcomes, and efficient resource use.

Minimum Qualifications
  • Current RN license in the state of practice.
  • Basic Life Support (BLS) certification for healthcare providers.
  • Current driver’s license, reliable transportation, insurance, and acceptable driving record.
  • Effective verbal and written communication skills.
  • RNs with less than 12 months of RN experience prior to hiring must obtain their BSN within four years of the hire date.
Preferred Qualifications
  • Bachelor's Degree in Nursing (BSN).
  • Previous experience in an Acute Care, Home Health, or Hospice setting.
  • BSN required as of the job description’s last update on 4/27/2025; employees hired or promoted prior to this date will remain subject to the requirements in place at that time.
Physical Requirements
  • Continuously see, read, and assess information, labels, and patient needs.
  • Engage in frequent verbal communication and listen to spoken information, alarms, and patient needs, especially during emergencies.
  • Use manual dexterity to manipulate complex equipment with precision, including frequent computer use and typing for documentation.
  • Lift and utilize full range of movement to transfer patients, bend to retrieve, lift, and carry supplies up to and including heavy items.
  • Walk and assist with transporting and ambulating patients, and distribute supplies and equipment, including pushing and pulling gurneys and portable equipment.
  • Stand in a stationary position for extended periods.
  • Drive a vehicle when required, watching traffic signals and signs, and ensuring safe operation.
