We are seeking a compassionate, well‑organized Home Health or Hospice RN who takes pride in delivering high‑quality care to elderly and frail patients.

Duties & Responsibilities
  • Develops and initiates the plan of care; assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver(s).
  • Initiates preventive and rehabilitative nursing procedures and provides services as ordered by the physician according to the plan of care.
  • Provides patient, caregiver, and family counseling and education.
  • Prepares clinical notes and updates the primary physician at least every sixty days.
  • Communicates with the physician regarding the patient’s needs and reports any changes in condition; obtains/receives physician orders as required.
  • Co‑ordinates with community health personnel to align the care plan.
  • Participates in the HHA’s quality assessment, performance improvement program, and sponsored in‑service training.
  • Participates in on‑call duties as defined by the on‑call policy.
  • Ensures arrangements for equipment and other necessary items and services are available.
  • Instructs, supervises, and evaluates home‑health aide care provided every two weeks.
  • Supervises LPNs/LVNs and/or HHAs/CNAs as directed.
  • Job Requirements
    • Graduated from an approved school of nursing and licensed in the state of practice.
    • Minimum of two years of experience, at least one year in public health or home‑care nursing preferred.
    • Current CPR certification.
    • Licensed driver with an insured automobile in good working order.
    • Self‑directed, able to work with minimal supervision.
    • Excellent observation, problem‑solving, verbal and written communication skills.
    • Ability to organize and prioritize workload independently.
    • Prolonged walking or standing; able to lift, position, or transfer patients; able to lift supplies and equipment; considerable reaching, stooping, bending, kneeling, or crouching; visual acuity and hearing required.
