Position Summary

Provides certified nursing assistant services to assigned patients in accordance with care plans, facility policies and procedures, and under the direction of supervisor(s).

Required Qualifications
  • Certified Nursing Assistant in good standing with the state (preferred, not required).
  • High school diploma or equivalent.
  • Current CPR/BLS certification.
  • Current Food Handler Permit (when applicable).
  • No disciplinary action in effect against certification.
  • Preferred: at least one year of experience as a CNA in a skilled nursing facility (SNF) or assisted living facility (ALF). Not required.
Responsibilities
  • Provides supportive services to nurse, med tech and other staff as needed and performs duties as assigned.
  • Assists resident with or performs activities of daily living (ADLs) in accordance with care plans and established policies and procedures.
  • Assists resident with lifting, turning, moving, positioning, and transporting into and out of beds, chairs, bathtubs, wheelchairs, and lifts.
  • Coordinates dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake.
  • Delivers nutritional supplements to patients at assigned times and provides assistance as necessary to ensure intake; documents intake accordingly.
  • Assists with weighing patients according to facility policy, records weight, and reports weight changes to nurse and supervisor.
  • Tracks the condition of the resident’s skin, reporting any pressure areas, skin breakdown or tears to nurse and supervisor.
  • Assists nursing staff in carrying out toileting program activities.
  • Completes flow sheets daily to indicate that the specified task was performed; documents the progress of each resident as directed.
  • Keeps nurse and supervisor informed of any factors that interfere with performing assigned work (e.g., resident refusal, re‑assignment).
  • Reports changes in a resident’s condition to the resident’s nurse and supervisor.
  • Promotes and protects all patients’ rights, treats all patients with dignity and respect, and maintains confidentiality of protected health information.
  • Reports noncompliance with policies, procedures, regulations or breaches in confidentiality to appropriate personnel; reports any retaliation or discrimination.
  • Participates in life safety and emergency drills and fulfills responsibilities during activation of the facility’s emergency plan.
  • Completes all assigned training and skill competency as a condition of employment.
  • Attends care‑plan meetings of assigned patients.
  • Participates in QAPI or facility assessment activities as needed.
  • Accepts CNA assignments as staffing needs require; performs CNA duties in accordance with facility policies and procedures.
  • Uses proper lifting and body mechanics while delivering care to patients.

  • Able to move intermittently throughout the workday; can walk, stand, sit, bend, stoop for extended periods.
  • Meets general health requirements per facility policy, including medical and physical exams and immunization checks.
  • Ability to work beyond normal hours, on weekends and holidays when necessary.
  • Ability to assist in evacuation of patients during emergency situations.
  • Ability to bend, stoop, kneel, crouch, perform overhead lifting and other common physical movements.
  • May be subject to falls, burns, odors, hazardous chemicals, infectious waste and emotional patients; must maintain professionalism.
  • Strong listening skills and conflict‑resolution abilities.
  • Basic computer skills, including navigation of electronic medical record systems.
  • Ability to lift, move, push or pull a minimum of 35 pounds.
