PRN Nurse Practitioner

• Maintain a current insurable driver’s license in the State of Utah (if operating UICSL vehicles).
• Strive to learn and serve in compliance with federal regulations and UICSL policies and procedures, and to seek solutions for issues or problems they encounter in alignment with those resources.

• Provide comprehensive primary care services including evaluation, diagnosis, treatment, and management of acute and chronic conditions within scope of practice.
• Perform comprehensive patient histories and physical examinations and develop individualized care and treatment plans.
• Provide preventive care services, screenings, immunizations, and health promotion counseling appropriate to patient age, risk factors, and medical history.
• Manage chronic diseases common to the population served, including diabetes, hypertension, obesity, and related conditions using evidence based clinical guidelines.
• Order, perform, and interpret laboratory and diagnostic tests within scope of practice and ensure timely follow up of results and treatment plans.
• Prescribe medications and therapeutic interventions in accordance with scope of practice and applicable regulations.
• Provide patient and family education to support disease prevention, treatment adherence, and self management of health conditions.
• Coordinate referrals and follow up care with specialty providers, behavioral health services, and community resources as appropriate.
• Collaborate with behavioral health, nursing, case management, and clinical support staff to support team-based care, continuity of services, and safe transitions of care within a patient centered medical home framework.
• Support whole person and family centered care by addressing medical, behavioral, and social factors that influence health and wellness.
• Maintain accurate and timely clinical documentation within the electronic health record in accordance with organizational standards.
• Participate in staff and interdisciplinary meetings, peer review, and quality improvement activities that support coordinated patient care and clinic performance.
• Maintain current licensure, certifications, and clinical competencies required to perform duties of the role.
• Maintains and honors patient and organizational confidentiality, ensuring all clinical activities comply with HIPAA, UICSL policies, professional standards, and applicable regulatory requirements.
• Promotes whole person and family centered wellness by delivering and coordinating care responsive to the medical, social, emotional, physical, and cultural dimensions of health for patients and families served by UICSL.
• Performs other duties within scope of practice as assigned by the Medical Director or Director of Clinical Operations in alignment with UICSL’s mission, vision, and community centered approach to care.

• Education:
  
  a) Master’s degree or higher from an accredited Nurse Practitioner or Physician Assistant program
• Experience:
  
  a) One (1) year of primary care experience preferred; qualified new graduates may be considered.
• Certification/Licensure: a) Must hold an active, unrestricted Utah licensure as an Advanced Practice Registered Nurse or
• Other: a) Must pass a mandatory criminal background check.

• Education:
  
  a) Minimum Qualification, and any additional clinical training or certifications supporting primary
• Experience:
  
  a) Two (2) or more years of primary care or community health experience
• Certification/Licensure: a) Experience working in I/T/U healthcare settings
• Other: a) National Board certification, and eligibility to obtain and maintain DEA registration preferred.
• Knowledge of evidence based primary care practices and chronic disease management
• Knowledge of community health and population health principles
• Knowledge of quality improvement and patient safety practices

• Skill in verbal and written communication with patients and care teams
