**** provide holistic, patient-centered care and facilitate smooth transitions between care
**** settings.**** - Educating patients and families about healthcare options, emphasizing palliative care
**** choices and advanced care planning.**** - Facilitating access to supportive services such as hospice care, social support, and
**** community resources, as required.**** - Upholding professional standards, following policies, procedures, and regulatory
**** requirements to ensure compliance and protection of the healthcare provider's practice.**** - Staying current with medical knowledge and palliative care practices by participating in
**** ongoing education, attending professional development opportunities, and staying
**** connected with relevant networks.**** - Enhancing the healthcare practice's reputation by promoting high standards of patient
**** care and seeking opportunities to provide additional value and support to patients and
**** families.**** - Providing coverage for other practitioners as needed within the care group.**## Qualifications
