Job Summary:

The Case Manager is responsible for working with a caseload of individuals and/or families as assigned by the Clinical Director. The Case Manager works closely with each client to address the basic needs of clients; and provides individualized case management and referrals to help clients achieve self‑sufficiency. The Case Manager is an active and integral member of the Clinical Team.

Job Responsibilities:

Direct Client Services 50%:
  • Meet one‑on‑one with clients on a regular basis to develop an individualized case management plan.
  • Provide clients with referrals to services in the community, ensuring that the referral is completed and client is connected.
  • Coordinate appointments for clients for services that are related to their therapeutic treatment. Work collaboratively with clients on goals that move the client toward self‑sufficiency.
  • Provide interpretation for services, and for related services outside the office when other interpreters are not available.
  • Provide transportation for clients as needed.
Indirect Client Services 40%:
  • Know and understand eligibility requirements and documentation for torture survivor and asylum programs.
  • Keep updated and detailed client files on each client in caseload and clear and concise case notes.
  • Collaborate with community partners, including case workers at refugee/immigrant serving agencies, to coordinate services for clients and avoid duplication.
  • Attend outreach events and present on agency services as needed.
Agency Responsibilities 10%:
  • Participate in other department and organization‑wide activities, meetings and trainings.
  • Complete administrative responsibilities.
  • Perform other duties as assigned
  • Maintain licensure/credentials as needed, including required CEU’s
  • Attend trainings relevant to job function, as supported by the budget and supervisor
Qualifications:

Required education, experience, certificates, licenses or registrations
  • Two or more years of experience providing direct services to culturally and linguistically diverse clients
  • Fluency in Arabic, Spanish, or another language spoken by THRIVE clients.
  • Ability to interpret for clients in a culturally sensitive manner.
  • Valid driver's license and current automobile insurance.
  • Must pass and maintain background check
Preferred education, experience, certificates, licenses or registrations
  • A four‑year college degree preferred or equivalent professional work experience.
  • Active membership or participation in local immigrant communities.

  • Experience, comfort, and sensitivity working in a multi‑cultural environment is required.
  • Demonstrated understanding of the US immigration system, including asylum process
  • Excellent written, verbal and interpersonal communications skills
  • Ability to maintain confidentiality, recognize appropriate boundaries and cope with emotionally stressful situations
  • Ability to work as a part of an interdisciplinary team and multi‑cultural team
  • Ability to work independently and effectively manage multiple priorities and one's own schedule
  • Flexibility to adapt to changing program requirements and political climate
  • Ability to work in a dynamic context and respond to various needs
  • Working knowledge of Microsoft Office suite (Outlook, Word and Excel) and Google Apps
  • Demonstrated interest in issues of immigrant communities, political torture, severe war trauma, social justice, and human rights issues

Supervisory Responsibilities:

No supervisory responsibilities; however, may oversee volunteers and/or interns.

  • Services are provided on‑site at THRIVE offices.
  • Time spent on the computer is approximately 50%
