Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.
As a private, non‑profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.
The Licensed Vocational Nurse (LVN) provides direct nursing care, participant monitoring, care coordination, and clinical support services under the direction of a Registered Nurse (RN) and/or Primary Care Provider (PCP). The LVN works collaboratively with the interdisciplinary team (IDT) to promote participant health, safety, independence, and quality of life in accordance with PACE regulations and organizational policies.
Schedule:
Monday-Friday 8am-5pm.
- Provide direct nursing care to participants in the PACE center, clinic, and/or participant homes as assigned
- Perform nursing assessments within the LVN scope of practice and report changes in participant condition to the RN and PCP
- Obtain and document vital signs, weights, blood glucose readings, and other clinical data
- Administer medications and treatments as prescribed and within scope of practice
- Monitor participants for medication effectiveness, side effects, and adverse reactions
- Assist with wound care and other nursing procedures as delegated
- Ensure infection control and safety standards are maintained
- Collaborate with the interdisciplinary team to implement participant care plans
- Communicate participant concerns, changes in condition, and care needs to the RN and other team members
- Assist with scheduling and coordination of participant appointments, referrals, and follow-up care
- Support transitions of care following hospitalizations, emergency department visits, or skilled nursing facility stays
- Provide health education and reinforce teaching provided by the RN, PCP, and other disciplines
- Educate participants and caregivers regarding medications, treatments, and disease management
- Promote preventive care and healthy lifestyle practices
- Document nursing care, observations, interventions, and participant responses in the electronic health record (EHR)
- Maintain accurate, timely, and complete clinical documentation in accordance with regulatory and organizational requirements
- Report incidents, accidents, and safety concerns per policy
- Comply with all federal and state PACE regulations, California nursing regulations, and organizational policies
- Maintain participant confidentiality and HIPAA compliance
- Participate in quality improvement and performance improvement activities
- Participate in interdisciplinary team meetings as requested
- Assist with participant clinic visits and day center activities as needed
- Respond to participant concerns and emergencies appropriately

- High School Diploma/GED Required
- Valid California Licensed Vocational Nurse (LVN) license required and is to be maintained as a condition of employment
- Current Basic Life Support (BLS) certification through an American Heart Association (AHA)-approved source is required upon hire and must be maintained as a condition of employment
- 1 year of nursing experience preferred
- Experience working with geriatric populations, chronic disease management, home health, long-term care, or PACE programs preferred
- AHA-approved courses include an in-person, hands‑on skills check with a certified instructor using a mannequin to demonstrate CPR and emergency response techniques
