Neighborhood Healthcare, a nonprofit health system, seeks a Licensed Vocational Nurse (LVN) for its PACE program in California. The LVN provides direct nursing care, conducts assessments, administers treatments, and coordinates with an interdisciplinary team to support participant health.
You will document care in the EHR, educate participants and caregivers, and assist with care planning and transfers. A CA LVN license and BLS are required; 1 year of nursing experience is preferred.
