Job Summary

The Lead Direct Support Staff are responsible for providing assistance in daily living tasks, personal care and support, and ensuring the personal safety and welfare of physically disabled, medically and behaviorally challenged individuals in a residential setting while serving as an effective role model.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Demonstrate knowledge, understanding, and implementation of the person’s individual service plan (IPP).
  • Model and use good decision‑making skills and access appropriate resources to assist in developing quality services.
  • Effectively implement person centered planning techniques.
  • Empathize with clients.
  • Maintain a high degree of confidentiality in working with clients and staff.
  • Model and display appropriate conduct during program hours.
  • Be respectful and considerate with family members, caretakers, neighbors, case workers and other involved individuals.
  • Implement household tasks and social activities as scheduled.
  • Maintain open communication with Supported Community Living Manager to inform team members on client progress with goals, medical and/or social needs.
  • Comply with documentation policies by submitting accurate and timely paperwork, including progress notes, billing sheets, timesheets, expense logs, and other required reports.
  • Maintenance of client funds. Ensuring receipts are recorded, filed, and maintained in a timely fashion.
  • Review and report client medical appointments to the Supported Community Living Manager, ensuring that client med sheets are up to date.
  • Ensure other DSP colleagues complete assigned duties, directing, and educating them when necessary.
  • Attend mandatory SCL staff meetings & in‑service training assigned by CROSSROADS, INC.
ADDITIONAL DUTIES AND RESPONSIBILITIES
  • Communicate effectively with the entire Supported Community Living team.
  • Abide by HCBS guidelines as indicated in the HCBS manual available to you per request.
  • Transport clients to and from medical appointments and outings around the community as needed.
  • Relay any concerns from both client and guardian to the Supported Community Living Manager as applicable.
  • Order prescription and non‑prescription medications for clients, if needed.
  • Check in medications from pharmacy and administer medication, First Aid, and/or CPR if necessary.
  • Awareness and follow-up by filling out request forms for maintenance and safety of the site.
  • Schedule vehicle maintenance as needed (site specific).
  • Inventory of first aid kits in the home.
  • Participating in an on‑call rotation.
Requirements

Required Training: Complete 16.25 hours of Relias training which includes DHS Dependent Adult Abuse training and HCBS Traumatic Brain Injury training. Complete and receive Safety‑Care training and Medication Management training within the first 6 months of hire.

MINIMUM QUALIFICATIONS
  • Must be able to work mornings, evenings, overnights, and weekends depending on the site needs.
  • Maintain current driver’s license and current auto insurance.
  • No found evidence of abuse of criminal activity.
  • Background checks showing no evidence of founded abuse or criminal activity.
Education

High School Diploma or GED is required.

Experience

Preferred 1 year work experience in the field of human services.

Knowledge/Skills/Abilities
  • Ability to meet physical and mental requirements necessary to perform all functions of position.
  • Ability to maintain confidentiality.
  • Communication and relational skills to interact in a professional manner with staff and clients.
  • Strong problem‑solving skills and time‑management skills.
Technology/Equipment

Ability to operate a variety of office equipment to include but not limited to personal computers, printers, and scanners. Working knowledge of Microsoft Office Suite software.

Physical Demands

Able to occasionally lift 40 lbs.
