RN Case Manager-Home Health

Overview

RN Case Manager – UnityPoint at Home. 1.0 FTE, 40 hours per week.

Schedule:

8:00am–4:30pm, with some call, rotating weekends and holidays. Sign-on bonus: $10,000.

• Provide one-on-one in-home patient care with total autonomy over the care plan from admission to discharge.
• Develop and oversee the home health plan of care under physician direction; ensure interdisciplinary involvement to achieve optimal patient outcomes and exceptional patient experience.

• Registered Nurse, licensed in state(s) where providing care.
• Minimum one year of nursing experience with a similar patient population.
• Valid driver s license and automobile insurance as required.
• Mandatory Reporter:
  Child & Dependent Adult Abuse.
• Person-Centered Care (PCC) course within 12 months of hire; annual competency validation.
• CPR/BLS certification with re-certification.
• Basic knowledge of commonly used medications, administration, actions and side effects.
• Competency in a range of nursing skills; ability to perform without supervision (e.g., NPWT, complex wound care, IV therapy, catheter care, ostomy care, chronic disease management, teaching).
