UnityPoint Health in Iowa seeks an experienced RN Case Manager to support UnityPoint s 1.0 FTE role offers 40 hours weekly, 8:00am–4:30pm, with some call, rotating weekends and holidays. A $10,000 sign-on bonus is included.
You will provide one-on-one in-home patient care with total autonomy over the care plan from admission to discharge, and develop and oversee the home health plan under physician direction to ensure optimal outcomes and patient experience.
