Registered Nurse (RN) – Rehabilitation – Days. Sign on Bonus: 10,000. Your experience matters. As a member of the Health Support Center (HSC) team, you will support those who are in our facilities, interfacing and providing care to our patients and community members to positively impact our mission of making communities healthier.

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides care appropriate to condition and age of the patient.
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

• Current state RN license required.
• Basic Life Support certification is required within 14 days of hire.
• ACLS and PALS certification required within six months of hire.
• One year of experience preferred.

Day 7:00am-7:00pm. Job Location Type:
