RN, Rehabilitation Unit
Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.
Reports to:
The Post Acute Director. FLSA:
Non-exempt.
- Associate’s degree required;
Bachelor’s degree preferred. - Graduate of a credited nursing program.
- Current RN license to practice in the state of AZ.
- Certifications:
American Heart Association BCLS. New employees with BLS certification from another organization (e.g., American Red Cross) have 30 days to complete the American Heart Association version.
- Critical thinking skills and decisive judgment.
- Ability to work with minimal supervision.
- Capacity to work in a stressful environment and take appropriate action.
- Performs plan of care interventions, including medication administration, specimen collection, clinical treatments, and other medical care treatments.
- Documents patient care provided.
- Monitors, records, and communicates patient condition to the care team, physician, patient, and family as appropriate.
- Assists patients with activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
- Collaborates across disciplines to coordinate patient care, including transfer, discharge, referral, and spiritual/psychosocial support needs.
- Evaluates learning needs of patient and/or family and provides education appropriate to age, culture, condition, and circumstances.
- Advocates for the physical and emotional well‑being of the patient.
- Assesses, plans, implements, and evaluates care for a population based on age‑specific and cultural needs in a collaborative multidisciplinary approach.
- Assures quality of care by measuring health outcomes against patient care goals and standards, making or recommending necessary adjustments, and following hospital philosophies and standards of care set by Arizona State Board of Nursing and other governing agency regulations.
- Coordinates the delivery of patient care for the Rehab Unit, facilitates problem solving with staff, and communicates necessary issues to nursing administration and the Program Manager.
